Home healthcare includes the following services:
The home health aide may assist periodically with bathing, hair and nail care, and other personal care. An aide is assigned at the discretion of the visiting nurse, but only if the patient is already receiving skilled nursing, physical therapy or speech therapy.
Our physical, speech and occupational therapists provide high-quality rehabilitation services in your home. These services include therapeutic exercise, functional activities, patient education and screening, evaluating and interpreting results. We also instruct your family in functional transfers, balance activities and stroke rehabilitation. Specific services include:

A social worker is available to assist patients with social or financial problems that may impact the patient’s care or recovery.
Baxter Health Home Health telemonitoring remotely monitors patients’ vital signs data and sends short surveys about current health statuses to the patients and caregivers. Clinicians can track patients’ conditions daily, which promotes fast follow-up and intervention.
